Dimenhydrinate Injection

What is Dimenhydrinate Injection?

Dimenhydrinate Injection is an antihistamine with significant anticholinergic and anti-emetic properties. It is a combination drug consisting of diphenhydramine and 8-chlorotheophylline. It is primarily used when oral administration is not possible to rapidly alleviate symptoms of motion sickness and vertigo, and to control nausea related to various medical conditions.

Uses of Dimenhydrinate Injection

  • Prevention and treatment of motion sickness (nausea and vomiting).

  • Management of vertigo and dizziness associated with Meniere’s disease or other labyrinthine disorders.

  • Control of post-operative nausea and vomiting (PONV).

  • Relief of nausea induced by radiation therapy or certain medications.

  • Management of vestibular disturbances affecting balance and orientation.

How Does Dimenhydrinate Injection Work?

Dimenhydrinate works by inhibiting histamine H1 receptors and cholinergic stimulation in the vestibular apparatus (the balance center of the inner ear). By dulling the sensitivity of the labyrinthine system and inhibiting the chemoreceptor trigger zone (CTZ) in the brain, it prevents the transmission of signals that trigger the vomiting reflex.

FAQs on Dimenhydrinate Injection

1. Can Dimenhydrinate Injection be administered intravenously? Yes, it can be given via deep intramuscular (IM) injection or slow intravenous (IV) injection. If administered IV, it should be diluted and injected over a period of at least 2 minutes to minimize the risk of sudden drowsiness or transient stinging.

2. Does Dimenhydrinate cause significant sedation? Yes, drowsiness is a very common side effect of Dimenhydrinate. Patients should be cautioned against operating heavy machinery or driving immediately after receiving the injection. The sedative effect is often more pronounced than with newer-generation antihistamines.

3. Is it safe for use in patients with glaucoma? Dimenhydrinate should be used with extreme caution in patients with angle-closure glaucoma. Because of its anticholinergic effects, it can increase intraocular pressure, potentially worsening the condition.

4. How long does the effect of one injection last? The onset of action is almost immediate following IV administration and within 20–30 minutes after IM injection. The therapeutic effects typically last for about 3 to 6 hours, depending on the patient's metabolism.
