Dipyridamole Injection

Dipyridamole Injection is a thrombolytic medication used to treat myocardial infarctions (heart attacks) as soon as possible (usually within 12 hours) after the onset of symptoms. It works by dissolving the harmful blood clots in the blood vessels, thereby restoring the blood flow to the affected tissue, preventing tissue death, and improving outcomes. Tenecteplase is given as an injection into the vein under the supervision of a doctor.

 

  • Description:

    • Dipyridamole is a coronary vasodilator, chemically described as 2,6-bis(diethanolamino)-4,8-dipiperidino-pyrimido-(5,4-d) pyrimidine.
    • It is available as an injection and is administered intravenously.
    • Each milliliter of Dipyridamole Injection contains 5 mg of dipyridamole, 50 mg of polyethylene glycol 600, and 2 mg of tartaric acid in Water for Injection, USP.
    • The pH of the solution is adjusted using hydrochloric acid.
    • It is used as an alternative to exercise in thallium myocardial perfusion imaging for evaluating coronary artery disease in patients who cannot exercise adequately1.
  • Mechanism of Action:

    • Dipyridamole acts as a coronary vasodilator.
    • Its exact mechanism of vasodilation is not fully understood, but it may involve inhibition of adenosine uptake, an important mediator of coronary vasodilation.
    • The vasodilatory effects of dipyridamole are abolished by the administration of the adenosine receptor antagonist theophylline1.
  • Clinical Pharmacology:

    • In a study, Dipyridamole Injection resulted in an average fivefold increase in coronary blood flow velocity compared to resting coronary flow velocity.
    • Cardiovascular responses include a mild increase in heart rate and mild decreases in systolic and diastolic blood pressure, returning to baseline values within approximately 30 minutes.
    • Dipyridamole-induced vasodilation may lead to abnormalities in thallium distribution and ventricular function, possibly due to a “steal” phenomenon where relatively intact vessels dilate, leaving reduced pressure and flow across areas of hemodynamically important coronary vascular constriction1.
  • Indications:

    • Dipyridamole Injection is indicated as an alternative to exercise in thallium myocardial perfusion imaging for evaluating coronary artery disease in patients who cannot exercise adequately1.
    • It is also used as part of a “stress test” to assess how well the heart is pumping blood2.
